Patients
When your physician recommends therapy, ask about the programs and services offered by Professional Therapy Services, Inc. (PTS). We are certified by Medicare and approved by most health insurance companies and provider networks.
To schedule an appointment you may contact any of our conveniently-located clinics. The locations and phone numbers are listed on our website under the heading Locations & Maps. Our staff
will assist you in scheduling a time and clinic location convenient for you. When you call, we will need information concerning your health insurance, current
diagnosis, and the type of therapy your physician has ordered for you. We can assist you by:
- Checking your insurance to determine if there is coverage for the requested therapy service and what will be covered.
- Identifying any non-covered expenses you may be responsible for and any information your insurance company may need to process payment.
- Completing forms for your insurance company or employer.
- Setting up a payment plan. Professional Therapy Services, Inc. does not charge interest, service charges or late fees. Our Billing Department can be reached by calling toll-free 1-888-236-9705.

For your convenience, we have attached the following patient registration forms to print and complete.
Please bring completed forms to your first therapy appointment along with you medical insurance card(s).
Physicians
We welcome the opportunity to serve you and your patients. For your convenience, we will provide your office
with preprinted physician order forms which contain the various therapy modalities, procedures, treatment protocols, and the
special services we offer. In addition, we have designed reporting forms to communicate therapy progress and the
treatment outcomes of your patients. At your request, communication is provided by mail, fax, phone, or
electronically following established confidentiality guidelines.
Employers
A number of educational, contractual, and injury management programs and services are offered for employers
and contract customers. Whether designing an injury prevention strategy, developing a return-to-work program,
or performing an ergonomic audit, each service is tailored to the unique needs of our customer.
